Tennessee Correctional Department Commissioner Frank Strada has announced his resignation following the failed execution of Christa Pike, the first woman to be executed in the state in over two centuries. According to a statement from Governor Bill Lee, a Republican, Strada will step down from his position at the end of October.

Tennessee Governor Strada has announced a decision to halt executions in the state following the failed lethal injection of inmate Pike on September 30. This incident marks the second failed execution in Tennessee this year, with the previous instance occurring in May when medical staff encountered difficulties in accessing a prisoner's vein, resulting in the suspension of the execution.

Tennessee Governor Lee expressed his gratitude to Strada for prioritizing the interests of the state's residents during a challenging period. The authority reaffirmed its stance that the Department has adhered to established protocols in fulfilling its responsibilities. In response, Governor Lee has decided to postpone executions until the end of the year, allowing for a review of the protocols in place.

A woman, who received two lethal injections, is currently in critical condition and hospitalized, unconscious and reliant on a respirator. Medical professionals are working to stabilize her condition and remove the pentobarbital from her system, a highly alkaline and caustic substance that can cause tissue damage and chemical burns.

A review of the legal documents reveals that the execution team employed at least seven needles in an attempt to administer the lethal injection, with one of the needles bending at a 90-degree angle during removal. Amidst the procedure, the inmate, Pike, reportedly provided verbal cues to the officials, indicating the correct placement of the needles, and was heard swallowing and gasping for air. She also expressed her distress, stating "I feel my arm is going to break" and inquiring "Is this happening like this?

A 50-year-old woman, who had spent nearly three decades on Tennessee's death row after being convicted in 1996 of murdering a fellow student, Colleen Slemme, was set to become the first woman executed in the state since 1819. The impending execution had sparked bipartisan calls from state lawmakers for the governor's resignation and a comprehensive overhaul of the state's death penalty system.
